Making a Life Insurance Claim (where the policy-holder has been diagnosed with a terminal illness):
- Nature of the diagnosed illness
- Date of diagnosis
- Predicted life expectancy
- Details of any treatment
- The contact details of the treating consultant

Making an Income Protection Claim (due to illness):

Making an Income Protection Claim (due to unemployment):
